24th Annual Alberta Amputee Open

2025 Alberta Amputee Open Registration
Please review all information listed below, then complete and submit your Registration to secure your spot in this event.
Registrations can be submitted either by:
(a) our Online (Web) Option, or
(b) our PDF below which can be printed off, filled out by hand, and then mailed (or scanned and emailed) to our event organizer.
ACCOMMODATIONS:
AASRA has booked a block of rooms for Friday, Saturday & Sunday (July 4-6) at the following hotel: MICROTEL INN & SUITES BY WINDHAM, 4915 - 49 Ave, Whitecourt, AB, T7S 0E8
There are 20 rooms blocked off for our event. Players must book directly with the hotel by calling the hotel at 1-780-396-0990. The rate will be $119.00 per night plus tax and fee, breakfast included. When reserving, mention you are with the AASRA golf tournament.
Note: The cut-off date for booking your room is June 19th, 2025. Late requests may be permitted at the same rate but are subject to availability.
REIMBURSEMENTS:
AASRA will reimburse Lifetime Amputee Members (who are in good standing for at least one year) for:
1) ACCOMMODATIONS:

Members living more than 150 km away will be able to book 2 nights hotel.

Members who live more than 250 km away may book 3 nights hotel.
Members will be able to claim back accommodation cost with receipts. Please refer to our AASRA Expense Claim Form on the website.
AASRA members may share a room if they wish but only one member can claim the room cost for expenses. If you have any questions concerning expenses, please contact the President via email.
2) TRAVEL:

Members living within 250km away will be able to claim up to $150.00 for gas with receipts.

Members who live more than 250km away, will be able to claim up to $200.00 for gas with receipts.
Please Note: AASRA members may claim back their entry fees by using their Rec Funding only if the member has Rec Funding still available.
Members are responsible to pay their own expenses and then submit a Funding Reimbursement Form.
Scanned copies of receipts must be submitted with this form to receive reimbursement. PLEASE DO NOT SUBMIT PHOTOS OF YOUR RECEIPTS.

We need people to assist at the course for registration, contacting your prosthetist or a business about becoming a hole sponsor or donating some prizes. The Alberta Amputee Sports & Recreation Association (AASRA) supports and provides opportunity for amputees in recreation and sporting activities, in both disabled and able bodied events. AASRA also provides peer support to new amputees and leads regularly scheduled support group meetings in Calgary and Edmonton. AASRA currently supports participation by amputee athletes in over 20 different sports.
The Alberta Amp Open is an annual event held at various locations around the province from year to year, and this year is our 24nd annual. We typically welcome approximately 40 attendees who compete over 2 days and 36 holes of golf, in multiple divisions including Stroke Play and Stableford.
